The All-Russian Union of Patients and the Social Mechanics Center for Humanitarian Technologies and Research provided Kommersant with the results of a study on the availability of innovative drugs. Only half of the surveyed Russians with severe disabling diseases receive free innovative drugs due to them – however, the proportion of respondents who are aware that they should be prescribed such drugs is about 53%, and a third of the respondents do not know about it. The path of an innovative drug from the moment when the patient learns about its existence to inclusion in the treatment regimen usually takes from one to three years.
The study “Availability of innovative drugs-2022” was conducted by the Center for Humanitarian Technologies and Research “Social Mechanics” at the request of the All-Russian Union of Patients. They interviewed 817 Russians with severe chronic, including orphan, disabling diseases from 80 regions. The authors clarify that these people need drugs to maintain their health and life, so they can be called one of the most immersed in the issue and aware of the achievements of drug therapy target groups.
The exact number of Russians with severe disabling diseases is unknown. Estimates of the number of patients with rare (orphan) diseases differ. Thus, according to the expert council on orphan diseases under the State Duma Committee on Health Protection, in 2020, about 15 thousand people were included in the state program to help people with rare life-threatening diseases, and more than 21 thousand people were in the program of 14 high-cost nosologies. Rarus magazine, which publishes the All-Russian Society for Orphan Diseases, speaks of 2-7 million people with rare diseases in Russia (not all such diseases have treatment). In addition, according to the Ministry of Health, in 2020, 3.7 million people with cancer lived in Russia: a significant part of these patients also need innovative therapy.
According to 86.4% of respondents, taking innovative drugs to some extent improves the patient’s quality of life: 58.5% of respondents indicated noticeable improvements, and 27.8% – small changes for the better. Among the characteristics of an innovative drug, the most significant patients consider the opportunity to get a better quality of life in general (50.2%), then the best therapeutic effect (47%) or the ability to cure the disease (43.5%). Reduction of adverse reactions in innovative drugs worries 40% of respondents. Comfort during treatment is a little less important – the ability to take pills less often or inject injections (35.9%), a longer period of remission or improvement in well-being (32.7%), a more convenient and less painful procedure for taking the drug (23.1%). A small number of patients noted the possibility of shifting monitoring and administration of the drug to the device and not choosing the time and method of drug administration (7.6%).
However, the availability of innovative drugs provided free of charge to privileged categories of citizens is not yet too high. Only 51.1% of patients believe that they receive the innovative drugs they are entitled to. Another 32.1% of patients report that they receive their free innovative drugs on time; received with delays – 19%. And 33.4% answered that the innovative drugs they were entitled to were not available. At the same time, the situation has improved compared to estimates in 2020. At that time, the participants in the study “Assessment of the availability of innovative medical technologies and medicines for patients in the Russian Federation” answered that innovative therapy is not available in 45% of cases.
Half of the respondents know that innovative drugs should be prescribed to them during treatment (53.2%). A third of patients do not have information about whether innovative drugs are included in the list of drugs prescribed by them (34.9%). In 11.9% of patients, such drugs are not included in the list of prescriptions.
The existence of innovative drugs according to the profile of their disease was noted by half of the respondents (49.2%). 39.0% do not have information or are not sure about the existence of such drugs. And 11.6% of respondents indicated the absence of such drugs. Half of the patients informed about innovative drugs say that they are not currently introduced in Russia. Indicate that according to the profile of their disease there are no drugs that are not used in Russia, 7.7%.
The path of an innovative drug from the moment a patient learns about its existence to inclusion in treatment in Russia is one to three years, according to 43.2% of the patients surveyed. Less than one year elapsed between receiving information about the drug and the drug itself in 31.8% of patients. Part of the patients currently receiving treatment with innovative drugs, they became available only after three to five years; more than five years after receiving information about him passed for 17.1%. According to experts, in 2020 drugs were included in the state guarantee program for one to three years in 27% of cases. Most often this happened within three to five years (in 41% of cases), and in 32% of cases the process took more than five years.
The survey also showed that only 23.3% of the respondents had no problems obtaining medicines. The remaining patients mention the absence of the prescribed drug in the pharmacy warehouse or pharmacy (26.4%); refusal of the attending physician to issue a prescription, prescription or medication (17.1%); refusal of the head of the polyclinic to issue an appointment or collect a medical commission (7.8%); refusal of the medical commission to prescribe the medicine (7%).
The survey authors also asked patients where they get information about innovative medicines (the choice was multiple). Medical organizations and medical personnel occupy an insignificant share among the sources, while the main source of information about the emergence of innovative drugs is the patient organization (85.7% answered this way).
Commenting on the survey, Yan Vlasov, co-chairman of the All-Russian Union of Patients, notes that now, for several reasons, the availability of innovative drugs has fallen. “Due to the geopolitical situation, logistics times are increasing, delivery is becoming more expensive. This cannot but affect the availability of drugs and consumables for diagnostic research methods,” explains Mr. Vlasov. Production in localized factories is difficult. To a greater extent, the situation has affected medical nutrition and, to a lesser extent, pharmaceutical products.” Jan Vlasov adds that clinical trials are completely closed, namely, they are an opportunity for patients with many chronic diseases to receive an innovative drug.
The scientific director of the network of clinics for immunorehabilitation and preventive medicine Grand Clinic Olga Shuppo notes another problem: medicine, at least private medicine, is faced with rising prices for equipment and consumables. “The transition to domestic analogues will not be a panacea for medicine – in the production of equipment, many consumables, the share of imported components is high,” says Ms. Shuppo. Yan Vlasov agrees with her: “Investments in science are not growing, they account for about 0.05% of GDP, which means that development in the pharmaceutical industry cannot be expected.”
