“Only by palpation can you understand that this is a prosthesis”

Samara doctors successfully performed a complex operation to replace the scrotum of a young man who is studying at one of the local universities. Such prosthetics made from medical silicone are carried out in cases where a man has one testicle instead of two in the scrotum.

“Such medical interventions are rarely carried out, but require a highly qualified surgical team,” Anton Ivanchikov, a urologist at the department of urology and organ transplantation at SamSMU clinics, told MK. – Testicular replacement is more of an aesthetic operation associated with the fact that men, for one reason or another, lack a scrotum or testicle. Well, so that you can go to the bathhouse with men or with a young lady without being noticed. From a physiological point of view, the prosthesis does not provide functionality. Roughly speaking, we insert a silicone prosthesis into the scrotum. This can be either a congenital defect or the result of various injuries or surgery. This is almost the same thing when a woman’s mammary gland is removed (mainly due to cancer) and a silicone prosthesis can be immediately installed so that the woman feels emotionally complete and beautiful. In this case, we do it for men.

Regarding this operation, our patient was a young man (he was 24 years old), he had one testicle. The operation itself lasted about an hour under spinal anesthesia. Then three days in the hospital and that’s it – we were sent home, and then we see the patient on an outpatient basis. Now the young man feels well and has already started studying. Externally, the testicles do not differ at all, and only a doctor upon palpation can understand that this is a prosthesis.

– So why are there such patients? What reasons?

– The guys played football, hitting and that’s it… Or fights, in which they could also lose a testicle and other consequences of domestic injuries. And also oncology. For prostate cancer (this is a hormone-dependent tumor) and after removal of the testicles, silicone prostheses are placed. But now there are fewer such patients, since the country is actively developing an oncology program in healthcare, and prostate cancer is detected at an early stage. Another thing is prosthetics of the male genital organ, which is done in connection with erectile dysfunction. There are one-component, two- and three-component silicone prostheses. After such an operation, a man may well become a father by conceiving a child, since an erection occurs with the natural filling of the penis and blood vessels with blood. And ejaculation occurs not through the prosthesis, but through the urethra. That is, after prosthetics, a man can live a full sex life and, among other things, conceive a child naturally. Such operations last up to two hours, depending on the components. Let me explain: three-component, this is when the patient presses on one testicle after surgery, the penis works, presses on the other, and lowers. Depending on your desire. This is a hydraulic system – blood is pumped from one reservoir to another.

– How will this man have sex, love?

– This is an artificial erection, as long as a man needs. Ejaculation will occur, but an erection will not occur. And after sex, he himself removes the erection. That’s all.
