The Social Fund, which from this year should accrue additional payments to Russian healthcare workers, will monthly request funds for this purpose from the Federal Compulsory Medical Insurance Fund. The Ministry of Health has developed an application form with which the Social Fund will receive funds for additional payments to medical workers in the Russian healthcare system – a draft order was published yesterday on the regulation.gov.ru portal. Recall that from this year the government will pay extra to a number of categories of medical workers – Vladimir Putin announced this in December 2022. Among them are doctors and nurses working in primary health care in the central district hospitals, in district hospitals and in the ambulance system. Depending on the position and place of work in 2023, they will be able to receive from 4.5 thousand rubles. up to 18.5 thousand rubles. monthly. As Deputy Prime Minister Tatyana Golikova said earlier, although health issues are supervised by the Ministry of Health, payments will be made by the Social Fund subordinate to the Ministry of Labor, into which the Pension Fund and the Social Insurance Fund have been merged since this year.
As follows from the draft order, the Social Fund will monthly submit an application to the FFOMS subordinate to the Ministry of Health, so that the latter transfers the funds that need to be paid to health workers this month. The new payments, as indicated in government decree No. 2568 of December 31, 2022, will be financed from federal transfers to the MHIF.
As a Kommersant source in the Social Fund explains, the choice of the latter as the operator of federal payments is due to the fact that the FSS, which has become part of this organization, already has such experience. So, in 2020, the FSS was instructed to organize the payment of “covid” surcharges to medical workers, after the performance of this work by the regions was criticized by medical trade unions (for more details, see Kommersant of May 7, 2020). “Then the FSS was able to demonstrate that the Sotstech IT system developed for it back in 2014 is suitable for this purpose and can provide fast and targeted payments,” Kommersant’s interlocutor notes. The current payments entrusted to the Social Fund will also be carried out with its help, as indicated in the government decree.
It should be noted that the model that the government has chosen to solve the problem of low salaries of medical workers – targeted selective payments – in the future can be extended to other categories of state employees. So, back in 2021, Vladimir Putin instructed the government to organize the collection of detailed statistics on the salaries of teachers, doctors and scientists, as well as to work out options for reforming their remuneration. However, the pilot project, which would significantly change the approaches to calculating their salaries, although it was developed by the Ministry of Labor and the Ministry of Health, subsequently did not receive the approval of the Ministry of Finance as too costly (for more details, see Kommersant dated September 21, 2021). As a result, the implementation of the pilot was delayed until 2025. The costs of the currently proposed targeted medical payments are about 150 billion rubles. per year, we note, will cost the budget about five times cheaper. As Kommersant’s source in the social bloc notes, in the face of limited budgetary resources, it is the approach in which the government takes on the burden of new social spending in the form of one-time or “special” payments – to doctors or, for example, pensioners – may be the most viable. “This allows us to improve the situation pointwise, without getting involved in long-term obligations that may turn out to be impossible,” he notes.
