In 2022, almost a third - 36 regions of the Russian Federation - did not provide the Unified Health Information System (EGISZ) with data on medical organizations and doctors necessary for the correct operation of the geoinformation map of Russian healthcare. At the suggestion of the Ministry of Health, the State Duma will consider government amendments on fines for the lack of such information for the heads of subjects. In most regions surveyed by Kommersant, the existence of a problem is not recognized or the problems with data transmission are explained by the difficulties in the operation of the regional digital circuit.

Almost a third of the regions of the Russian Federation in 2022 did not enter data into the Uniform State Health Information System subsystems. To force them to do this, the Ministry of Health has developed a draft amendment to the Code of Administrative Offenses on appropriate fines for the heads of regions where information exchange with the federal Ministry of Health is sabotaged.

The USHEIS itself was launched in 2018, its main components and principles of operation are regulated by government decrees, and subsystems cover various sectors of the provision of medical services. For patients, the most demanded part is the federal electronic registry and an integrated electronic medical record with information about all diagnoses, medical services provided and medical certificates. The Ministry of Health, in turn, also considers the Uniform State Health Information System as a source of important information for coordinating the actions of the regions - the healthcare system in the Russian Federation is managed at the regional level, and failures in information exchange do not allow the federal center, among other things, to build full-fledged reporting. So, as one of the subsections of the Uniform State Health Information System, it was planned to create a geoinformation map of resources with a display of medical institutions and doctors on the territory of the Russian Federation. Since 2018, work on the Unified State Health Information System has been carried out as part of the federal project "Creating a single digital contour in healthcare based on the Unified State Health Information System", which is part of the national project "Health".

As follows from the explanatory note to the draft amendments, 36 regions of the Russian Federation (including the Moscow region, Crimea, Karelia, the Yamalo-Nenets Autonomous Okrug, etc.) in the first, second and third quarters of 2022 were not transferred to the federal register of medical organizations (FRMO) and the Federal Register of Health Workers (FRMR) data that are needed to build a geographic information map. As noted in the explanatory note, these data are necessary to achieve the "goals of rational territorial planning in the field of healthcare", and compliance with the procedure for transmitting information by regional ministries of health will improve the quality of medical care. To increase the responsibility of the subjects of the Russian Federation, the Ministry of Health proposes to supplement the Code of Administrative Offenses with a new Art. 6.37, which will allow Roszdravnadzor to fine the heads of regions if data from the local Ministry of Health does not enter the system on time or at all. The amount of the fine will be from 15,000 to 25,000 rubles; in the event of a repeated violation, the perpetrator will have to pay up to 30,000 rubles. or may be disqualified for a period of one to two years.

However, as follows from the answers of the regions from the list of the Ministry of Health that were interviewed by Kommersant, there were objective reasons for the difficulties with data transfer - or there were no problems. So, in the Komi Ministry of Health, “Kommersant” explained that the republican information and analytical system in the first to third quarters of 2022 did not have services for integration with the Uniform State Health Information System. According to the regional department, in the fourth quarter, the region began to use a new single digital platform, which made it possible to link it with the Uniform State Health Information System. The Ministry of Health of Yakutia explained that the medical institutions of the republic worked on systems that made it possible to fill in the necessary registers, and the data of organizations were entered into a single system individually.

On the contrary, in the Amur Region, Crimea, Sevastopol and YaNAO, the problems described by the federal Ministry of Health are not recognized. For example, the YNAO Health Department told Kommersant that, based on the results of the first nine months of 2022, their region ranks sixth in the Ministry of Health’s digital maturity rating, which implies “full interaction with all EGISZ services.” “Medical organizations of Yamal since 2016 have been filling in data in the FRMR and FRMO,” they emphasized. In the Ministry of Health of Crimea, Kommersant also said that they did not allow any failures in data transmission on their part: “Crimean medical organizations are constantly working in the federal register of medical organizations and the federal register of medical workers,” the press service specified.
